Sending a form to a patient to complete is very simple. You can either send it from the diary by left clicking on their appointment, and selecting Send Form from the options, or if it is an existing patient, you can go to their Details screen and click Send from the left side menu.

With either of those options select the form you want to send, then click Send.  An email will be sent to the patient with a link that they can click to be taken to the form page.

Once the form is completed, the form will be processed in one of two ways. For existing patients, the form will be saved as an entry in the clinical records.

For new patients who haven't been fully entered into the system using the "Enter details" option, the form will be saved until their details are entered. At that point, the system will match their details with the fields on the Enter details popup, and any extra fields will be saved in the patient's clinical records.
